Notice to TNDAGC Employees:
To avoid duplicate information, do not mail or fax your own individual Registration Form. Instead, give your completed Form to the District Administrative Assistant or Child Support Administrator. The Administrative Assistant in your District will collect all Registration Forms and submit them, with payment, to this office.
